New patient forms
Advanced Orthodontic Studio is completely digital! After your new patient consultation is scheduled, we will email you with a link to your forms. We ask that you click on the link to complete the online forms. We love that this technology saves you time, and gives us all the information we need to get you started with your treatment on the initial visit!

We respect your time and know that it’s incredibly valuable! Our goal is to be sure that, no matter how we meet you and your family for the first time, you are treated like a rockstar at all times!
